For each question, please choose the answer that best represents how you’ve been feeling over the past two weeks. Then, bring this checklist with you when you talk to your healthcare professional. Remember, depression is diagnosed by a doctor when a patient demonstrates at least 2 weeks of depressed mood or loss of interest accompanied by at least 4 additional symptoms of depression that interfere with functioning.
